At HCCD, we created Health Center Florida to help expand access to education, prevention, and care across communities throughout the state. Through this program, we work alongside local partners to support health access in ways that are responsive, respectful, and rooted in community needs.
Health Center Florida reflects HCCD’s broader commitment to advancing health access through shared care. It serves as a platform for connecting individuals to services, resources, and care navigation—while remaining grounded in local collaboration and trust.
Who we are
HCCD exists to advance health access through community care.
We focus on education, prevention, and respectful support as the foundation for healthier outcomes. By meeting people where they are and removing unnecessary barriers, we help individuals make informed choices about their health without judgment.
Our work is community-based and partnership-driven. We collaborate with local organizations, educators, and health professionals to deliver practical, culturally aware support that reflects the realities of the communities we serve.
We recognize that access is not one-size-fits-all. Some people need information, others need testing, guidance, or connection to additional care. By addressing these needs together, we help create pathways that support individuals, families, and communities over time.
As HCCD continues to grow, we remain guided by transparency, collaboration, and the belief that community care is the foundation of lasting health access.
